If you are considering a transfer from another hospice agency, you are most likely considering it for two main reasons. In this case, it could be directly related to either the fact that you are relocating outside of the hospice coverage area or that you dislike the services provided by your current hospice provider.
Can you transfer from one hospice to another? Yes, we are pleased to inform you that you are able to move from the old hospice you were in to a new hospice of your choice. You are at liberty to choose the hospice that you believe is the best for you and your family. You can switch hospices once per benefit period.
When you are transferring from one hospice agency to another, you should make sure to file a clear and signed statement with both the hospice agency that will transfer the patient, as well as the hospice agency that will receive the patient. There are three things a statement must specify: the hospice that preceded the new hospice, the date when the new hospice takes effect after the previous hospice, and the date on which the transfer took place.
