Hospice Valley provides outpatient hospice care under a community-based model. The offices in which we operate do not provide treatment or services. We, on the other hand, visit our clients in the comfort of their homes otherwise known as outpatient hospice. With this type of service, the patient remains in familiar, comfortable surroundings, free from the stress and discomfort of being in an inpatient setting. The outpatient hospice staff is available to assist you with any care you need at home so that you, your family, and your loved ones will be as comfortable as possible.

The ability to live at home and receive outpatient care before undergoing a hospital stay can offer people with terminal illnesses a unique alternative to hospitalization. It is important to mention the fact that hospital beds are in short supply and hospital stays can be quite expensive. An individual who stays in a hospital bed for a few days, or perhaps even a few weeks, will incur considerable costs. Besides such unhygienic conditions, hospitals can also be seen as unsocial, unfriendly places, where strangers enter and leave constantly, so it’s difficult for patients to feel comfortable.

In Hospice Valley, our mission is to provide you with all the help you need and to enable you to manage your symptoms at home in an outpatient setting.  We offer routine health care, as well as emergency care and other urgent needs.  Aside from the routine nursing visits, a 24/7 nursing support service is also available for any and all nursing needs.  Hospice Valley also employs a multidisciplinary approach, so we can provide you with outpatient certified nursing assistants, social workers, chaplains, volunteers, nurses, counselors, and services such as pet therapy to make sure your needs are being met.

Eligibility for Medicare’s Outpatient Hospice Benefit

In order to qualify under Medicare so that hospice services are covered, you must meet the following requirements:

  • Patient must be 65 years or older
  • Diagnosed with a terminal illness
  • Certification from a doctor that he or she has six months or less to live
  • Agrees to forgo life-saving or potentially curative treatment
  • Hospice provider must be Medicare-approved
